Cohort effect in mortality in Russian regions: economic losses

Abstract

This article analyzes the regional differentiation of the cohort effect in mortality from six major classes of causes of death in Russia. Based on cause-specific mortality data from 1989 to 2022 from the Russian Fertility and Mortality Database (RusFMD), age–period–cohort (APC) models were constructed for 76 Russian regions. The analysis shows that the strongest cohort effects among men and women born between 1970 and 1985 are observed in mortality from infectious diseases and diseases of the digestive system. Using the APC model results, excess mortality associated with elevated cohort effects for these cohorts was estimated. Economic losses from this excess mortality, measured as foregone benefits using the potential years of life lost (PYLL) methodology, were calculated for the period 2005–2022. Total losses exceeded 500 billion rubles, equivalent to 0.43% of Russia’s GDP in 2022. Infectious diseases, circulatory system diseases, and digestive system diseases account for the largest share of these losses for both sexes, while external causes of death also contribute significantly to excess mortality among men. The most affected regions are industrial areas that experienced severe economic hardship during the 1990s crisis, highlighting the importance of living conditions during formative years as a determinant of adult mortality.
